Uintah Basin Healthcare has a variety of financial assistance options available to our patients who are uninsured or underinsured. We will assist you determining if you qualify for financial assistance or if there are programs available that may help pay for your medical services.
Uintah Basin Healthcare Financial Assistance Policy
Financial assistance applications are available in all clinics, patient admitting areas, patient financial advocate offices, on our website, or by phoning our Patient Financial Services at (435) 725-2060. Once we receive your completed application, we will review the information to determine what assistance may be available to you and notify you of the outcome within 10 business days.
