Medical

Uintah Basin Medical Center (UBMC) offers two choices in health care coverage.

  • Both plans cover the same services, but have different deductibles, co-pays, and paycheck deductions.
  • Both health care plans offer a wide range of services, including but not limited to, office visits, hospitalization, maternity, emergency care, surgery, mental health and prescription drugs.

Health Savings Account (HSA)

  • HSA’s are accounts created exclusively to pay for qualified medical expenses. Accounts may be funded with pretax dollars up to a maximum of $3,450 for an individual with single coverage on a high deductible health plan (HDHP) or $6850 for a family HDHP.

Flex Spending Account

  • A Flexible Spending Account (FSA) provides a method for employees to pay for dependent care or medical/dental expenses with pre-tax dollars through payroll deduction.
  • Healthcare Spending Limit: $2,650
  • Dependent Care Spending Limit: $5,000

Dental

  • Preventative care is covered at 100% when using a network provider.
  • Ability to see any dentist you choose.
  • Wide range of covered services, including but not limited to, fillings, oral surgery, crowns, bridges and orthodontics.
  • Low out-of-pocket cost when receiving care from an In-Network dentist.

Life Insurance and AD&D

  • $40,000 life insurance policy and $10,000 for dependents paid for by UBMC.
  • Matching amount of accidental death and dismemberment benefit.
  • Supplemental life insurance may be purchased up to five times salary with a maximum of $400,000.

Long Term Disability Insurance

  • Pays up to 60% of the first $5,000 of your pre-disability earnings.
  • Disability benefit is paid by UBMC.

Short Term Disability Insurance

  • Short term disability insurance may be purchased by the employee.
  • Pays us to 60% of weekly income to a maximum of $1,000.
  • Benefits begin on the 14th day for disability due to an illness or accident.

Employee Assistance Program

  • Offers professional, confidential counseling services and referrals.

Paid Time Off (PTO)

  • PTO is earned on the number of hours paid (up to a maximum of 80 hours per pay period) and by length of service.
  • PTO can be used for vacations, illness, and personal time.

Extended Illness Bank (EIB)

  • Employees accrue a specific amount of time per pay period.(Employees working for the Villa do not accrue EIB.)
  • EIB can be used for extended illness, jury duty, or bereavement.

Holidays

  • Observes seven holidays.
  • 1 floating holiday.

Education Reimbursement

  • Education reimbursement is available to full time or part time employees after six months of employment subject to certain criteria.

401K Retirement

  • Employees must be at least 21 years of age and have been employed for six months.
  • Employer match and tax savings.
